CT & MRI Findings in Ankylosing Spondylitis of the spine

Ankylosing spondylitis (AS), in the spine can involve the anterior and posterior elements. It usually commences anteriorly and as the disease progresses, posterior changes are seen. In these images we look at:

  • Corner erosions and Sclerosis. 

  • Syndesmophyte formation between vertebrae.

  • Fusion: Of some of the posterior spinal joints.

WHAT ARE THE FINDINGS?

The CT demonstrates sclerosis and some rounding of the corners of the vertebral bodies in keeping with Romanus Lesions in Ankylosing Spondylitis. Shiny corners refers to the sclerosis seen on CT.

CT Spine changes in Ankylosing Spondylitis:

Some early changes involving the vertebral body in ankylosing spondylitis:

    • Corner Erosion (Pink arrow)  & Sclerosis (Yellow arrow),

       

    • Fusion (Green arrow),

       

    • Syndesmophyte (Blue arrow).

WHAT ARE THE FINDINGS?

  • This is a T1 weighted scan.

  • There is bilateral facet joint fusion and ankylosis (no joint space seen) in a patient with ankylosing spondylitis.

  • The high (white) signal seen in the facets is a chronic reactive change of fatty change post inflammation.

WHAT ARE THE FINDINGS?

  • CT in a Patient with chronic spinal Ankylosing Spondylitis.

  • There is CostoTransverse (Green arrows) & CostoVertebral Fusion (Pink arrows).
